Adults living with chronic conditions face a variety of challenges and many of those challenges can threaten their independence. If your loved one is living with a health condition that makes daily activities difficult, Bayview Hunter's Point Adult Day Health  Center can help.

Participants attend daily programs at our center. We arrange transportation to our facility for Medi-Cal beneficiaries and the  cost is covered. Family members can then use this time to pursue careers, handle household chores or simply rest.  We offer sliding scale fees for those unable to qualify for Medi-Cal.

Services include personalized therapies to help your loved one stay as active and independent as possible. This care plan is reviewed frequently to make sure it still meets his or her needs.

Bayview Hunter's Point Adult Day Health Center is dedicated to finding solutions that make a difference in the life of your loved one or patient.

You may contact us with your referral by telephone from 9 am to    4 pm, Monday-Friday.  Please ask to speak with a social worker, or leave a message on extension 1006 or 1013.
